For all donation and sponsorship requests, you will need to complete the Donations/Charitable Giving Request Form.

Only requests including this form will be reviewed and must adhere to the overall mission statement of the committee as included within the application. Please be sure to include any pertinent information that will assist us in the review process. If the application is incomplete or insufficient, the committee will return the form for the necessary information which may delay processing.
Email forms to ldowning@thefirstnational.com or you may drop off them off directly to Larissa Downing at the main desk during regular business hours. Deadline for all forms will be the last day of every month. Applicants will be notified by the 15th of each month of determinations and donations issued accordingly.
